Healthy Families Initiative

 The Healthy Families Initiative is a collaborative effort of Healthy Mothers, Healthy Babies Coalition of Wake County and Strengthening The Black Family, Inc. The goal of the Healthy Families Initiative (HFI) is to improve the health behaviors and birth outcomes for identified families involved in HFI project activities.

Project Activities

1. To maintain and provide training opportunities through HFI partners for a collaborative network and services to families of childbearing age (men and women ages 18-44) mostly affected by poor birth outcomes.
   
2. To provide health education, life skills development, case management, social support and information and referral to 50 families of child bearing age (men and women ages 18-44) mostly affected by poor birth outcomes.
   
3. To increase knowledge of infant mortality and morbidity and community resource assistance to address these issues.
   

Program Components

1. The educational meeting is to provide information to participants in gaining knowledge to facilitate gaining knowledge to improve the family’s health and well-being.
   
2. The support group is to create a relaxed comfortable environment in which women collectively share and deal with “life” and societal issues that undermine their well-being.
   
3. Case management is a support service provided to connect women with resources and information to improve their quality of life.
   
4. Childcare and meals are provided at the support and educational meetings.
   

How You Can Help.

1. Refer participants to the program.
   
2. Volunteer as a mentor to a family.
   
3. Share information on health topics related to the projects goals.
   
4. Serve on a special committee or HFI Working Group.
   
5. Make a donation to help cover program expenses.
